The Missouri Foundation for Health (MFH) has identified the increase in the prevalence of obesity among Missourians as a serious public health issue in the entire MFH service region. The Healthy & Active Communities program will target community-based organizations with the specific objective of reaching women and children, racial and ethnic groups, and low income populations who are at increased risk of developing obesity. Funding will be available to community agencies demonstrating the capacity to promote collaboration and community participation. The MFH will fund projects that entail direct program implementation, community education, improved community access and development of local public policy that addresses obesity prevention. Funded projects will include those involving public and private collaborations, as well as individual agencies that have the ability to implement model programs that support efforts to enhance the health and activity of local communities. For the purposes of this initiative, the Foundation expects a focused definition of "community," (e.g. a single school district, town, or county).

Collaboration is a key strategy in maintaining programs over time. To promote this stability, funded agencies will be expected to participate in building local collaboratives/coalitions to address obesity prevention. These collaboratives/coalitions will jointly plan and take action to achieve health changes, increase efficiency and decrease duplication of services. Grantees will be expected to work with Foundation-funded consultants to achieve effective collaboration.

Because the University of Missouri is limited to one letter of intent for this program, letters of intent must be submitted to the Office of Research, 203 Jesse Hall, no later than 5:00 pm Monday, April 25, 2005. Letters of intent submitted to the Office of Research should be the same as the one the applicant will make to the MFH if selected, in accordance with specifications outlined in the MFH guidelines.

Amount: $150,000 per year, for 2 years
MU Deadline: Monday, April 25, 2005
MFH Deadline: April 29, 2005 (letter of intent)
